Joint Record Form
The form used by health visitors, school nurses etc to notify the joint agency team of children who meet the threshold for a Joint Record (at least one area of difficulty at a moderate level).
Outcome
- Child's details entered on record and a copy sent to parents/carers.
- Parents/carers will be contacted by the Referal Co-ordinator to see if they wish to receive an information pack and regular newsletters etc.
- Parents/carers can contact Referal Co-ordiantor for information/advice.
CSN1 - Initial Information Record
This does not constitute a referral - it is used to make the joint agency aware of a particular child's circumstances. Professionals have to ensure that the child meets the threshold for a joint record.
Outcome
- Advice or emergency services can be given at this stage.
- The parent/carer will be sent a copy of the CSN1.
- If the child is not already on the Joint Record the parent/carer will be asked if an appropriate professional can check this with them.
Self referral
You can refer your child yourself to your local Joint Agency Service by completing a CSN1 form.

CSN2 - Information Gathering Form
If the Referal Co-ordinator considers an integrated assessment may be required, parents/carers will be contacted to see if they will complete a CSN2 - professional support will be arranged by the Referal Co-ordinator if required.
Outcome
- Receipt of CSN2 constitutes a referral which is discussed at the next Joint Agency Service Consultation/Allocation Meeting.
Joint Agency Service Consultation/Allocation Meeting
Outcome remain on Joint Record only:
- request to single agency for further assessment
- advice to single agency to enable them to retain case management
Outcome integrated assessment required:
CSN3 (assessment summary)
- initial keyworker allocated.
- CSN3s requested from appropriate professionals.
CSN4 (integrated assessment and service plan)
- CSN4 completed.
- key worker agreed with parents/carers.
CSN5 (service plan revue)
- CSN4 will be reviewed at least 12 monthly to ensure that services are adequately meeting the requirements of the child and family.
