Before you leave care, your care manager or key worker will explain what will happen.  They will try to ensure that by the time you leave care:

• you are well prepared for independence
• you leave care when YOU are ready, not before
• you are not alone
• you have enough money to manage properly.

BUT whether the process works or not depends on you, not your care manager or key worker.
