CAADA DASH RIC (Risk Identification Checklist) and Risk Management Pathways for Devon

Guidance for Medium Risk following completion of the adva & CAADA DASH RIC

If you have ticked between 6 - 13 ‘yes’ boxes the case would currently meet the Medium Risk criteria. If you have ticked less than 6 - 13 ‘yes’ boxes but it is your professional judgement that this is a ‘Medium Risk’ case you are dealing with, the case would also currently meet the Medium Risk criteria.

A reminder about ‘Medium Risk’:

‘Medium Risk’: There are identifiable features of risk or serious harm. This level of risk should be referred to the local specialist domestic violence and abuse ‘Outreach’ service in Devon (but this can only be done with signed consent on the completed RIC). Make the referral to ‘Outreach’ services once you have obtained the necessary signed consent on the RIC. Explain that nobody needs to live with domestic violence and abuse and that there is support out there. It is important to remember that risk is dynamic and should be monitored. It is also important to stress that the police can and should be contacted in an emergency. Hand out the adva ‘Home is Where the Hurt is image - PDF icon (28KB - pdf help)’ domestic abuse services leaflet with appropriate contact numbers for your area and ask the person to keep you informed about their safety, especially if they haven’t given their consent for a referral. Encourage them to make a self-referral in this instance.

‘Medium Risk’ = Professional judgement and / or the number of ticks’ on this CAADA DASH checklist. If you have ticked 6 – 13 ‘yes’ boxes the case would currently meet the ‘Medium Risk’ criteria.

Medium Risk Referral

There are 3 specialist domestic violence and abuse ‘Outreach’ services in Devon:

Stop Abuse for Everyone covers Exeter, East and Mid Devon

Domestic Violence and Abuse Service South and West Devon covers South Hams, Teignbridge and West Devon.

North Devon’s Women’s Aid covers North Devon and Torridge.

Please complete the Medium Risk referral form appropriate to the specialist domestic violence and abuse ‘Outreach’ service in your locality:

Please send your completed Medium Risk referral form, together with your completed CAADA DASH RIC (including signed consent) to your local specialist domestic violence and abuse ‘Outreach’ service (details on the referral form).

Referrals should be sent by secure email (encrypted) or other secure method. No form of post (registered or otherwise) is considered secure anymore for sharing personal information.

Sharing Information Securely

Sending personal information by fax:

Do not fax personal or confidential information unless it is absolutely necessary.

Secure (encrypted) email is the safest way to share personal or confidential information securely.  If you do not have access to a secure (encrypted) email, it is possible to fax such information providing you follow the procedures below carefully:

  • Telephone the recipient to ensure that they are aware a confidential fax is about to be sent, to confirm that an identified individual will collect and deliver it and that safe receipt will be confirmed.
  • Send a test sheet with ‘Please Ring Me to Confirm Receipt’ if this is the first time using the fax number.
  • Mark the cover sheet ‘Strictly confidential, intended for the addressee only. In the event of an error please contact the sender immediately’.
  • Information should be restricted to the minimum necessary and only those items that are essential to the purpose of the disclosure. Information should be anonymous where possible to limit identification of the service user when it is not relevant.
  • Use pre-installed numbers wherever possible to minimise the risk of misdialling. Double check the fax number before sending.
