What Each Agency Is Expected To Do

County Special Education Team (CSET)

Year 8    Inform Connexions, Health & Social Services of statemented young people.

Year 9    Send out invitations to year 9 transition review for young people educated other than at school.

Year 10    Inform relevant agencies of annual review dates for pupils educated out of school/non-maintained/independent schools.

Year 11    Inform relevant agencies of annual review dates for pupils educated out of school/non-maintained/independent schools.

School

Year 8    Inform Connexions of young people on School Action & School Action Plus

Year 8    Prepare for year 9 transition review

Year 9    Send out invitations to year 9 transition review

Year 9    Consult with parents as to other professionals involved

Year 9    Attend year 9 transition review

Year 9    Draw up transition plan

Year 10    Inform relevant agencies of annual review dates

Year 10    Prepare for and attend year 10 review

Year 10    Research post 16 opportunities/options available in relationship to the young person’s identified needs

Year 11    Inform relevant agencies of annual review dates

Year 11    Prepare for and attend year 11 review

Year 11    Check progress of transition plan

Keyworker

The keyworker will probably be a representative of another agency (eg JAT, Connexions, school), and will therefore be responsible for that agency’s role as well as the role of the keyworker.

Year 8    Prepare for year 9 transition review

Year 9    Attend year 9 transition review

Year 9    Co-ordination/overseeing delivery of transition plan

Year 10    Prepare for and attend year 10 review

Year 11    Co-ordinate information to parents/professionals prior to review

Year 11    Prepare for and attend year 11 review

Year 11    Review transition to college/employment/day services

Year 11    Review long-term independence/housing

Year 11    If it appears that a young person might be eligible for adult services, then a ‘pre-referral’ should be made to the appropriate adult service team so that they may consider appointing a shadow care manager

Year 11    If young person leaves school, inform other agencies named in transition plan of new post-16 learning provider, and co-ordinator of next review

Year 12    Inform relevant agencies of annual review dates. Information to parents/professionals prior to review

Year 13    Review of transition plan

Connexions PA

Year 8    Prepare for year 9 transition review

Year 9    Attend year 9 transition review

Year 10    Prepare for and attend year 10 review

Year 10    Research post 16 opportunities/options available in relationship to the young person’s identified needs

Year 11    Prepare for and attend year 11 review

Year 11    Choices – applications or funding applications

Year 11    Review transition to college/employment/day services

Year 11    If leaving school, Section 140 assessment completed and passed on to post-16 provider and other relevant agencies with consent

Year 11    Agree likelihood of future joint involvement/Connexions only support

Year 13    Review of transition plan

JAT Team Member

Year 8    Prepare for year 9 transition review

Year 9    Attend year 9 transition review

Year 9    If there is a strong possibility of significant input/funding required at 18+, inform the relevant adult services team

Year 10    Prepare for and attend year 10 review

Year 10    If there is a strong possibility of significant input/funding required at 18+, inform the relevant adult services team

Year 11    Prepare for and attend year 11 review

Year 11    Agree likelihood of future joint involvement/Connexions only support

Year 13    Review of transition plan

Year 11    Core information and possible future needs passed on to adult services
