Planning for your future
The planning for your future will normally start at your first review after you turn 15, although you may be older than that now.
With your help, we will look at your education or training, relationships and practical skills. If you wish, you can fill in a Self Assessment Form to help you think about what you want to do. Someone will help you fill it in if you want or you can use a computer.
By now, you should have an idea about what your goals are and what help you need to achieve those goals. You can record your decision in a Pathway Plan which may include:
- the training or qualifications you need to get the job you want
- the practical things other people do for you at the moment that you'll have to do for yourself
- your thoughts on some relationships and how you may be able to make them better
- when you think you may be ready to leave care (most young people are not ready before they are 18).
Your Pathway Plan records what you plan to do and what other people are going to do to help you. The Plan will be reviewed every six months until you are 21.
